Hennis Joe Supported Living and Outreach Services

Hennis Joe Care and Support is a provider for young people and adults 0-17,18 -64 and 65+ with Autism, Learning and Physical Disabilities, Drug, and Mental health. We support individuals to develop the abilities they need to move on to the next stage in their lives.

We specialise in supporting young people and adults towards greater independence and the provision of outreach support.
To put the needs of the clients we support at the centre of our care plan and ensure their involvement as well as their families in our decision-making process.

Preparing for Adulthood

Preparing for adulthood can be an exciting time for young people and their families and carers. 

Young people should enjoy increasing levels of independence and choice in their lives. Getting the transition process right will provide young people with positive goals and plans as they move into adult life. If it goes wrong there can be confusion, anxiety and disappointment, leading to poor outcome in later years.

Our client group range from learning disability issues through to complex needs involving autism and mental health

Supported living

Supported Living Services enable individuals with a disability or mental health to live independently in their own home, with appropriate support to help them manage their own tenancy and achieve freedom and control in their lives. 

We specialise in supporting young people and adults towards greater independence and the provision of outreach support.

The services we provide can range from 24hrs a day to Outreach Support for young people transitioning to adulthood. We create a unique and tailored personal support plan for each individual.
